Khadija Poitras- Rhea has been named Executive Director, Care Coordination & Population Health Management for Saint Francis Healthcare Partners. Khadija joined SFHCP in 2015 as the Director of Care Coordination and Population Health Management.

As a Licensed Clinical Social Worker, she has over eight years of supervisory experience and a focus on program development and expansion. Khadija has experience in the coordination of interdisciplinary teams, training/staff development, clinical interventions and cultivation of relationships with key stakeholders.

In her role as Director of Care Coordination and Population Health Management she has successfully implemented a Post-Acute Care Management program and Acute Inpatient Transition of Care program.

Prior to joining SFHCP, Khadija spent five years with Vinfen Corporation of Connecticut as the Senior Program Director where she played an integral role in the oversite for nine residential programs funded by the Department of Developmental Services.

Khadija earned her Masters of Social Work from Smith College, Northampton, Mass., and her BS in Human Services at Springfield College, Springfield, Mass.
